Children & Youth Participation Form

New Forms Required for Children/Youth

Correct personal, emergency and medical information is needed to make sure that Wellington Square United Church looks after children and youth in a responsible and safe way.
Participant and Medical information forms will need to be updated annually, just as many schools or sporting leagues require an annual update. Many changes can happen throughout a year, including changes in the home and health. These forms will be kept confidential and for quick access in an emergency.
Please return the forms to the attention of Cheryll Sullivan, Administration Leader, at the church. A drop off slot is available in front of the GraceLand office.
